We are seeking a compassionate and motivated Qualified Mental Health Professional (QMHP) to provide Community Support and Case Management Services for adults living with mental health and co-occurring substance use disorders. The QMHP plays a critical role in helping clients access essential resources, overcome barriers to treatment, and achieve greater independence and stability. Position Summary The Qualified Mental Health Professional (QMHP) provides community-based case management and rehabilitative support services designed to improve clients' overall functioning, promote recovery, and connect individuals with community resources. This position collaborates closely with therapists, psychiatric providers, substance use counselors, families, referral sources, and community agencies to ensure comprehensive, person-centered care. Essential Responsibilities Complete comprehensive psychosocial and case management assessments. Develop individualized service plans in collaboration with clients and the treatment team. Coordinate care with therapists, psychiatric providers, primary care providers, hospitals, probation officers, and community agencies. Link clients to community resources including: Housing assistance Employment services Educational resources Food assistance Transportation Financial assistance Social Security and Medicaid resources Medical providers Vocational rehabilitation Peer support services Assist clients with appointment scheduling and follow-up. Monitor progress toward treatment goals and recovery objectives. Conduct community outreach and client follow-up as appropriate. Provide psychoeducation related to wellness, recovery, coping skills, and community living. Advocate for clients and help reduce barriers to treatment. Maintain timely, accurate, and complete documentation in the Electronic Health Record (EHR). Participate in interdisciplinary treatment team meetings. Maintain compliance with HIPAA, HHSC, CARF, and agency policies. Qualifications Required Qualified Mental Health Professional (QMHP-CS or QMHP certification/eligibility under Texas requirements) Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, or a related behavioral health field Knowledge of serious mental illness, substance use disorders, and recovery principles Strong organizational and documentation skills Excellent interpersonal and communication skills Ability to work independently and as part of a multidisciplinary team Reliable transportation and valid driver's license (if community visits are required) Preferred Experience providing community support or targeted case management Behavioral health or substance use treatment experience Knowledge of community resources throughout the Greater Houston area Experience with Electronic Health Records (EHR) Crisis intervention experience Bilingual (English/Spanish) is a plus What We Offer Competitive compensation Flexible scheduling Supportive and collaborative leadership Professional development and continuing education opportunities Opportunity to grow with an expanding behavioral health organization Meaningful work that changes lives every day Why Strategic Therapeutic Solutions?
