Polk Mental Health Clinic- Livingston, TX (serving Polk, San Jacinto & Tyler Counties)
Position Number:
P #218 About the Role Burke is seeking a Mental Health Clinician (QMHP) to provide rehabilitative mental health services to adults, and potentially children/adolescents and their families, in both clinic and community settings. This role focuses on supporting individuals with chronic mental illness through individualized recovery plans while promoting stability and independence. What You'll Do Provide direct mental health services in office and community/home settings Deliver services in accordance with individualized recovery plans Conduct emergency assessments as needed Travel throughout Polk, San Jacinto, and Tyler Counties to serve clients Maintain accurate and timely documentation in compliance with Medicaid and Center standards Collaborate with clinical team under the supervision of a Clinical Coordinator Support individuals in developing skills for recovery and independence Perform additional duties as assigned What We're Looking For Bachelor's degree in Social Work, Psychology, Rehabilitation, or related Human Services field (must meet QMHP requirements) Valid Texas Driver's License Reliable personal vehicle with insurable driving record Strong organizational, time management, and documentation skills Ability to work independently and manage a flexible schedule What Helps You Stand Out Six (6) months of experience in a behavioral health or mental health setting Experience working with adult populations Familiarity with DSM diagnostic criteria LBSW Key Skills Clinical assessment and intervention Client engagement and relationship building Time management and self-direction Documentation accuracy and compliance Adaptability and problem-solving Work Schedule- Monday-Friday, generally 8:00 AM
- 5:00 PM Occasional after-hours work may be required Salary
