Exempt Position Summary The Licensed Professional Counselor (LPC) provides evidence-based behavioral health services within an integrated primary care setting at Hope Family Health Center. The LPC delivers brief, solution-focused interventions , supports real-time patient care through warm handoffs , and collaborates closely with medical providers to address the behavioral, psychological, and social determinants of health. This role is central to Hope's mission to provide whole-person, accessible care to underserved populations in the Rio Grande Valley. Key Responsibilities Clinical Care Provide brief behavioral health interventions (e.g., CBT-informed, solution-focused therapy) in a primary care setting Conduct same-day consultations ("warm handoffs") from medical providers Perform behavioral health assessments , including risk assessments (SI/HI, safety planning) Deliver short-term, goal-oriented therapy aligned with IBH best practices Manage a limited outpatient panel for follow-up visits Integrated Care Collaboration Collaborate with PCPs, nursing staff, and care teams to coordinate patient care Participate in team huddles and case discussions Provide consultation to medical staff on behavioral health concerns Support chronic disease management through behavioral interventions Documentation & Compliance Document all patient encounters in the EMR in a timely and compliant manner Maintain accurate clinical records in accordance with: Texas Behavioral Health Executive Council standards HIPAA and organizational policies Complete required outcome tracking and quality metrics Crisis & Risk Management Assess and manage patients presenting with: Suicidal ideation Homicidal ideation Abuse or neglect concerns Initiate appropriate escalation protocols , including coordination with PCP, emergency services, or higher levels of care Program Development & Community Engagement Contribute to development of Hope's IBH program Participate in outreach, education, and community-based initiatives Support grant-related activities and reporting as applicable Qualifications Required Master's or Doctoral degree in Counseling or related behavioral health field Active, unrestricted Licensed Professional Counselor (LPC) license in the State of Texas In good standing with Texas Behavioral Health Executive Council Minimum 1 year of clinical experience (preferred in outpatient or primary care setting) Preferred Experience in integrated behavioral health (IBH) or primary care settings Training in brief intervention models (CBT, Motivational Interviewing, SBIRT) Bilingual (English/Spanish) strongly preferred given patient population Core Competencies Strong clinical assessment and diagnostic skills Ability to deliver brief, high-impact interventions Team-based care mindset and interprofessional collaboration Cultural humility and experience working with underserved populations Excellent communication and documentation skills Work Environment Outpatient, community-based primary care clinic Fast-paced, team-oriented setting with real-time patient needs Frequent interruptions requiring flexibility and adaptability Physical & Operational Requirements Ability to sit, stand, and move between exam rooms throughout the day Ability to respond to urgent clinical situations Regular and reliable attendance requiredJob Type:
