OPEN UNTIL FILLED JOB SUMMARY
: The Qualified Mental Health Professional II - Transitions provides supervision and independent living instruction for residents with serious and persistent mental illness in a transitional home environment on a 24/7 basis. Work is performed under regular supervision and is guided by specific, well-established agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.
ESSENTIAL FUNCTIONS
: Instructs/Assists residents in basic independent living skills including: mobility, hygiene and self-care, home maintenance and safety, nutrition and home economics, social behavior and communication and functional academics, personal banking and money management, employment, etc. Performs supportive and administrative tasks to include but not limited to: Assistance with meal preparation, kitchen clean up, running errands for individuals, preparing grocery lists, picking up supplies/medications/groceries, assisting with daily facility clean up, leads residents in social and leisure activities, provides transportation, and assists residents with physician appointments. Dispenses medication and completes appropriate documentation in EHR. Records daily progress notes and communicates individual's behaviors to oncoming shift. Provides supportive level counseling and refers higher level needs to appropriate staff members as necessary. Acts as liaison with family members or other concerned parties on behalf of the individual after appropriate consent is obtained. Provides timely and accurate documentation of Individual Service Plans, telephone contacts, funding requirements, DBHDS requirements, and other information to meet program, agency licensing, and Medicaid billing requirements. Assists in the relocation of residents when appropriate.

QUALIFICATIONS
: Good oral and written communication skills. Knowledge of mental health, substance abuse, and ID symptomatology. Knowledge of treatment modalities and case management approaches, such as service coordination, behavior management, independent living skills training, supportive counseling, family education, and discharge planning. Knowledge of crisis intervention strategies and procedures. Ability to implement and lead individual and group training sessions, focusing on increasing recovery and self-determination. Knowledge of mental health, ID, and substance abuse confidentiality guidelines Valid Virginia Driver's License without any points Upon hire, individual will successfully complete CPR/First Aid Training, Medication Administration training, and Safety Care in order to maintain employment. EXPERIENCE/EDUCATION REQUIRED
: A "Qualified mental health professional" means a person who has (i) completed, at a minimum, a bachelor's degree. After hire, the candidate must complete the required approved training and education pathways for QMHP-T and QMHPs, including registration with the Virginia Department of Health Professions Board of Counseling https://www.dhp.virginia.gov/media/dhpweb/docs/counseling/policies/ApprovedTraining_QMHP_QMHPT.pdf Valid Driver's license and safe driving record required. Experience working with individuals with serious or persistent mental illness preferred.
