Qualified Mental Health Professional (QMHP/QMHP-T) Mental Health Skill-Building Services (MHSS) SpecialistPosition Description Under the supervision of the Clinical Director and designated Licensed Mental Health Professional, the Qualified Mental Health Professional (QMHP) or Qualified Mental Health Professional-Trainee (QMHP-T) provides Mental Health Skill-Building Services (MHSS) to adults diagnosed with serious mental illness in accordance with Virginia Medicaid, DBHDS, and DMAS regulations. The purpose of MHSS is to assist individuals in developing, restoring, and maintaining independent living skills necessary for successful community integration and recovery. Services focus on reducing functional impairments, increasing independence, preventing hospitalization, and supporting long-term recovery. Services are delivered in the individual's home and community using person-centered, recovery-oriented, trauma-informed practices. Minimum Qualifications Bachelor's degree from an accredited college or university in Social Work, Psychology, Counseling, Human Services, Sociology, Criminal Justice, or another Board-approved human services discipline. Current Virginia registration as a QMHP or QMHP-Trainee. QMHP-Trainees must remain under approved supervision until achieving full registration. Valid Virginia driver's license. Reliable transportation. CPR and First Aid certification. Successful completion of required background screenings. Essential Duties and Responsibilities Conduct intake and orientation sessions. Develop individualized service plans in collaboration with the LMHP and client. Implement person-centered interventions identified in the ISP. Teach independent living skills, including: Meal planning Grocery shopping Medication management support Personal hygiene Household management Budgeting Banking Employment readiness Time management Transportation training Community navigation Health and wellness Assist individuals in increasing psychiatric stability. Reinforce coping skills identified in treatment. Support medication adherence through education and routine development. Assist individuals in accessing medical, psychiatric, housing, vocational, educational, and community resources. Provide care coordination with treatment providers. Maintain regular communication with referral sources. Participate in ISP reviews and quarterly reviews. Monitor client progress toward measurable goals. Complete referrals and monitor outcomes. Provide transportation when clinically appropriate and authorized. Assist with discharge planning. Participate in interdisciplinary team meetings. Attend supervision. Attend all required training. Perform other duties assigned.
