is seeking a compassionate Behavioral Health Case Manager to support individuals receiving mental health and substance use disorder services in Anchorage, Alaska. The Case Manager will provide Intensive Case Management (ICM) and care coordination services designed to help participants access behavioral health treatment, medical care, housing, benefits, employment, transportation, and other community resources. Responsibilities Assess participant needs, strengths, barriers, and available supports. Assist with development and implementation of individualized service and treatment goals. Coordinate behavioral health, substance use, medical, housing, employment, benefits, and community services. Help participants access appointments and community resources. Monitor participant progress and identify barriers to treatment and recovery. Collaborate with clinicians, SUD counselors, peer support specialists, medical providers, families, and community agencies. Provide crisis support and appropriate referrals within the employee's scope of practice. Complete timely progress notes and other required documentation. Participate in treatment-team meetings, case consultations, and care coordination. Maintain HIPAA compliance, confidentiality, professional boundaries, and participant rights. Qualifications Bachelor's degree in social work, psychology, behavioral health, human services, or a related field preferred . Relevant behavioral health, substance use disorder, case management, or community-services experience preferred. Experience working with individuals with mental health, substance use, or co-occurring disorders is highly desirable. Knowledge of community resources, housing programs, public benefits, and recovery supports is a plus. Strong communication, organization, documentation, and problem-solving skills. Ability to successfully complete applicable Alaska background-check and credentialing requirements. Ability to meet Alaska Medicaid and 1115 Waiver requirements applicable to the assigned position.
