Job Description
Case Manager- Supportive Housing
Tuesday-Saturday 12:00pm-8:00pm.
Responsibilities:
The Case Manager will provide services to consumers in our supportive housing program.
Provide transportation for consumers as needed
Teach basic living skills
Assist consumers in accessing appropriate community resources as needed
Updating client records using an electronic clinical record system
Requirements:
BS Degree in Psychology, Social Work or other Human Services related field
Must have an excellent driving record free from any DUI/Reckless driving conviction for the previous (3) years. Will be subject to initial and ongoing MVR.
Must be 21 years or older
Must be willing to complete Case Management Training through the Alabama Departmental of Mental Health and obtain certification
Previous case management experience is preferred but not required
