Schedule:
Monday through Friday, 8am-5pm. North Alabama Community Care (NACC) is actively hiring a Maternity Case Manager in Madison County. NACC is a part of the Alabama Coordinated Health Network - a program of the Alabama Medicaid Agency. Our work supports Medicaid recipients by identifying and addressing issues and barriers that impact health outcomes for pregnant recipients and their babies. Requirements:
Must have a license related to field of study. For example, social work, nursing, counseling. Must be licensed to work in the state of Alabama (see Education requirements); Have strong verbal and written communication skills with the ability to encourage and engage recipients in plan of care; Ability to sufficiently document electronic case records including writing effective care plans and SMART goals; Ability to prioritize and manage a large caseload; Ability to work independently and collaboratively; Reliable transportation (local travel required) with active driver's license and insurance; and Works and lives within the counties of Cullman, Jackson, Limestone, Madison, Marshall, or Morgan. Education/Experience Bachelor's degree in a health science, social or behavioral health area of study.
Examples include:
Bachelor of Science in Nursing (BSN) degree with appropriate license; Bachelor of Social Work (BSW) from an accredited school of Social Work and appropriate license; Bachelor of Counseling from an accredited school of Counseling and appropriate license; or Bachelor of Social Work (BSW) or MSW from an accredited school of Social Work and appropriate license; -OR- Licensed Registered Nurse (RN) with an Associate Degree in Nursing (ADN) and two (2) years of recent experience working in a clinic setting or combined hospital experience; and A minimum of one (1) year experience in care coordination/case management working with low-income and diverse populations; must include experience working with individuals in the specific Medicaid population(s) receiving care coordination/case management services. Primary Responsibilites Provide and coordinate the care management services of Medicaid recipients, to increase quality of care and improve health outcomes. This includes, but is not limited to: Develop and evaluate person-centered care plan, working with recipients during the care management process; Identify needs related to health conditions, assess health literacy, and provide education on health conditions; Identify other needs and connect recipients with community resources; Perform other case management activities including but not limited to health risk assessments, social determinants of health screenings, attending medical appointments with the recipient, updating medication list, assisting with provider referrals and directives, linkage to a medical home, etc.; Conduct home visits and non-home encounters with recipients, recipients' family/caregiver, and/or support network; Collaborate with providers and healthcare professionals to ensure delivery of appropriate care and timely follow up care, creating sustainable clinical-community linkages to improve recipients' access to care and fill any identified gaps in services; and Provide consistent communication to NACC team members to evaluate recipient/family status, provide necessary follow-up and document progress in case record.
