Lifeline Professional Counseling is licensed as an outpatient treatment facility, focusing on providing comprehensive counseling services, including individual, couples and family counseling, as well as DBT, anger management, domestic violence classes and intensive outpatient services.
Details:
Full-Time, Monday-Friday, 9am-6pm. Starting pay is $20/hour, full benefits package.
Summary:
We are seeking a dedicated Behavioral Health Case Manager to join our team. This pivotal role contributes to our mission of delivering exceptional mental health care by overseeing counseling programs and ensuring quality service delivery.
Position Summary:
The Behavioral Health Case Manager provides care coordination and support services for children, youth and families. This role ensures clients are connected with the appropriate services, facilitates collaboration between systems and maintains accurate documentation. The case manager actively engages families, promotes service participation and supports overall treatment goals in line with agency and Mercy Care requirements.
Key Responsibilities:
Facilitate and coordinate Child and Family Team (CFT) meetings. Enter referrals into ClinicTracker and Excel tracking sheets. Prepare and submit referral reports to TERROS and La Frontera every other Friday. Contact new referrals to schedule services and ensure timely follow-up. Conduct intake process, including diagnostic evaluation, intake paperwork and coordination of initial services. Complete ESCII assessments, CALOCUS assessments and annual updates for ongoing clients. Develop ad update Individualized Service Plans (ISPs). Submit referrals for additional services and coordinate with community providers, schools and DCS. Complete and maintain CFT notes, case documentation and compliance paperwork. Submit AHCCCS 17+ report every 5th of the month. Actively engage clients who are not consistently attending services to support re-engagement. Attend required trainings and maintain knowledge of best practices in behavioral health. Provide advocacy for clients and families across systems of care. Support crisis response and coordination as needed. Act as a liaison between families, providers and systems to ensure continuity of care. Call to
