Job Description
Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for Medicare patients in a primary care setting. In this role, you will complete biopsychosocial assessments, develop treatment plans, and deliver brief, evidence-based interventions to address depression, anxiety, substance use, and other behavioral health needs. You will collaborate closely with primary care providers, nurses, and care managers, support crisis intervention and safety planning, and connect patients to community resources.
