As a Case Manager, you will collaborate with multidisciplinary teams, manage complex cases, and facilitate discharge planning to support patients' recovery and well-being. This paid position is ideal for dedicated healthcare professionals eager to make a meaningful difference in patients' lives through proactive case management. Responsibilities Conduct comprehensive patient assessments to identify needs related to medical, social, and behavioral health factors. Develop personalized care plans that align with clinical guidelines and patient preferences, incorporating discharge planning and resource coordination. Collaborate closely with physicians, nurses, social workers, and other healthcare providers to ensure continuity of care across hospital, outpatient, and community settings. Manage utilization review processes by evaluating medical necessity and appropriateness of services using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Facilitate communication with insurance providers for managed care approvals, prior authorizations, and claims processing. Monitor patient progress, adjust care plans as needed, and document all interventions thoroughly following HIPAA regulations. Provide education to patients and families about treatment options, medication management, and post-discharge resources to promote self-care and recovery. Skills Proven experience in critical care environments such as ICU or emergency medicine settings. Strong hospital experience with knowledge of acute care procedures and hospital medicine workflows. Expertise in nursing practices combined with familiarity with CPT coding, ICD-9/10 coding systems for accurate documentation. Experience working within Level I or Level II trauma centers or pediatric intensive care units (PICU). Knowledge of hospice care principles, discharge planning processes, and behavioral health considerations. Proficiency in EMR/EHR systems with an understanding of HIPAA compliance for patient privacy. Excellent communication skills for effective intake assessments and patient service delivery. Ability to navigate complex medical terminology confidently while managing utilization review and management tasks. Familiarity with managed care policies, utilization management protocols, and Level I trauma center standards.
