Behavioral Health Case Manager / Group Facilitator Opus Recovery Center Tempe, AZ Job Details Full-time | Contract $19 - $25 an hour 1 day ago Benefits Opportunities for advancement Qualifications Driver's License Driving Working with individuals with mental health conditions Full Job Description Minimum Qualifications Applicants must meet one of the following: Bachelor's degree in Behavioral Health, Psychology, Social Work, Counseling, Human Services, or a related field plus at least two (2) years of behavioral health experience; Associate degree in a related field plus at least two (2) years of behavioral health experience; High School Diploma/GED with four (4) years of progressively responsible behavioral health experience in case management, behavioral health, substance use treatment, or related services. Preferred Qualifications Experience facilitating behavioral health, psychoeducational, relapse prevention, CBT, DBT, or recovery-focused groups. Knowledge of AHCCCS behavioral health services and community resources. Experience working with co-occurring mental health and substance use disorders. Familiarity with trauma-informed care and Motivational Interviewing. Experience completing clinical documentation and electronic health records. Experience coordinating care with probation, parole, courts, medical providers, and community agencies. Peer support certification Bilingual (English/Spanish) preferred but not required. Essential Responsibilities Facilitate daily psychoeducational and recovery groups utilizing evidence-based curricula. Provide individualized case management and care coordination. Develop and update individualized service plans. Complete referrals for housing, employment, transportation, medical care, psychiatric services, food assistance, vocational services, and community resources. Coordinate services with therapists, medical providers, probation/parole, DCS, DES, AHCCCS health plans, and other community partners. Conduct client check-ins and monitor treatment progress. Assist clients with goal setting, problem solving, and recovery planning. Document all services accurately and timely in the electronic medical record. Participate in multidisciplinary treatment team meetings. Maintain professional boundaries and confidentiality in accordance with HIPAA and 42 CFR Part 2. Support clients using trauma-informed, recovery-oriented, and person-centered practices. Required Licenses & Certifications Valid Arizona Driver License with an acceptable driving record. Current Arizona Level One Fingerprint Clearance Card (or ability to obtain before hire). Current CPR/First Aid Certification (or ability to obtain within 90 days). Ability to pass a criminal background check and pre-employment drug screening. Knowledge, Skills & Abilities Strong verbal and written communication skills. Excellent organizational and time management abilities. Ability to facilitate engaging and educational groups. Knowledge of behavioral health documentation standards. Ability to work independently and collaboratively. Professionalism, integrity, and strong ethical judgment. Computer proficiency including Microsoft Office and Electronic Health Records (EHR). Schedule Full-Time Monday - Friday Day Shift Some community-based client appointments and transportation may be required.
