At Purpose Healing Center, we believe recovery is strongest when clients feel supported not only in therapy, but in the real-life details that make healing sustainable. Our Case Managers play a vital role in walking alongside clients as they navigate treatment, build stability, and imagine a life with purpose. We're looking for a Behavioral Health Case Manager / Group Facilitator who brings empathy, organization, and a steady presence to their work - someone who understands that healing often happens in the small moments: helping coordinate services, facilitating meaningful groups, and ensuring clients feel seen, safe, and supported. About the Role As a Case Manager, you'll provide direct support to clients while collaborating closely with our Clinical and Operations Teams. You'll help coordinate care, monitor client progress, and ensure that treatment and aftercare plans translate into real-world support. This role is ideal for someone who enjoys working directly with clients, values teamwork, and understands the importance of communication, documentation, and ethical care in a behavioral health setting.
Compensation:
$47,000 - $52,000, depending on your experience What You'll Do Support Clients in
Their Recovery:
Provide day-to-day support services to clients while they are in treatment Monitor client behavior and progress, documenting activities accurately and thoroughly Report any unusual behavior or safety concerns promptly to the Clinical Director Help ensure a positive, supportive client experience throughout treatment
Coordinate Care & Services:
Assist clients with outside appointments and services, including medical, legal, housing, aftercare, and scheduling needs Coordinate care between the Clinical Team and external service providers Support clinical and admissions staff as needed to ensure continuity of care
Facilitate Growth & Engagement:
Facilitate groups as assigned, supporting therapeutic goals and recovery education Offer guidance to clients around recovery models and healthy coping strategies
Maintain Standards & Compliance:
Uphold the highest standards of ethical and professional conduct Maintain strict confidentiality at all times Comply with all Purpose Healing Center policies, State and Federal regulations, and Joint Commission standards Complete documentation and reports accurately and on time What You Bring Required 21 years of age or older High school diploma or GED Ability to pass required background checks Knowledge of basic computer software (email, word processing, internet, spreadsheets) Strong interpersonal and communication skills Ability to prioritize tasks and maintain accurate client records Preferred At least 1 year of experience working in behavioral health Group facilitation experience Valid and insurable driver's license Possession of, or ability to obtain, a Level One Fingerprint Clearance Card (for residential programs) Why Purpose Healing Center?
