Tuesday-Saturday Essential Duties:
Provides addiction counseling in both individual and group environments, meeting with clients on assigned caseload individually on a weekly basis to address addiction issues and facilitating didactic and process groups per the clinical schedule Assists the client with addressing case management needs (housing, employment, etc.), including coordination of any legal related concerns Effectively communicates with families, referral sources, and relevant third parties when consents permit, providing clinical recommendations, progress updates, and continuing care recommendations Responsible for developing continuing care plans that support clients in achieving our mission, in addition to completing all relevant documentation (Discharge and Transition Plan, Discharge Summary, etc.) Meets with new clients while in detox and provides Orientation to the program Completes required documentation in a comprehensive and timely manner, always being mindful to document to medical necessity Enforces rules of the facility and implements consequences when needed Maintains client pass requests, submitting to the multidisciplinary team for approval at weekly meetings Provides crisis intervention The case manager is an active participant of the multidisciplinary clinical treatment team Limits personal disclosure Provides crisis intervention including accompanying transfers for psychiatric stabilization Provides coverage and back-up support for groups and individual sessions Participates in Family Group as needed Addresses recurrences of use and supports client in safe placement where needed and until completed Other duties as needed and/or assigned
Skills/ Special Requirements:
CADC I Minimum, CADC II preferred In network experience preferred Residential Treatment experience preferred Bachelor's Degree in related field is preferred Minimum of 2 years' experience in the field is required Must possess current California driver's license or Identification Card. Must possess or obtain CPR and First Aid Certifications within 15 days of accepting the position Minimum of 1 year of verifiable sobriety, if in recovery TB test to be conducted yearly or be verified by a physician.
