Criminal Justice Full Job Description Summary:
The Service Coordinator Team Lead is responsible for delivering quality mental health, substance abuse, and justice-involved supportive services face-to-face to an adult population. The Team Lead provides Behavioral Health recovery-based services consistent with the principles of Psychosocial Rehabilitation, Strengths-Based Models and Multi-System Team (MST), which may include, but are not limited to, assessment contribution, service plan development, case management, housing and benefits support, skills building, group facilitation, crisis intervention, and individual and/or group therapy and documenting all services. This position is community-based, with services provided in both office and field settings based on client needs. guidance, mentorship, and support to the team. The Team Lead acts as a resource and liaison between team members and Supervisors, ensuring smooth daily operations, consistent service quality, and adherence to clinic protocols. This position focuses on peer mentorship rather than direct managerial oversight, The Team Lead reports potential problems/issues within the program to the Program Supervising Team. The Team Lead collaborates with the team and supervisors in a positive solution focused manner contributing to the overall mission of providing comprehensive and compassionate care to clients.

Mentor & Training Support :
Serve as one of the point of contacts for complex cases, resource navigation, case management challenges, procedural questions, providing guidance and support to team members. Provide Mentorship and training support to the team, ensuring that they are delivering high-quality services in accordance with organizational standards and contract requirements. Mentor and support team members in their professional development.
Client Support :
Carry a caseload of clients w/ high complex needs, providing direct services including assessments, treatment planning, case management, crisis intervention, and documentation. Facilitate collaboration with external service providers to ensure holistic support for clients. Assist clients in navigating community resources, social services, housing, and financial assistance programs. Advocate for clients to ensure they receive appropriate and timely services. Meet with clients regularly per client needs and contract obligations. Regularly review and assess clients' progress toward treatment goals, adjusting social rehabilitation and case management services to achieve positive outcomes.
Assessments :
Assess and gather client information for domain-based assessments to evaluate the mental health needs of clients, considering factors such as medical history, psychosocial background, and presenting symptoms. Screen for housing, benefits, and community needs. Other assessments may include using the Strengths Based Model Assessment, ANSA, or other screening tools based on program requirements.
Treatment Planning:

Crisis Intervention:
Manage and respond to crises effectively, employing crisis intervention techniques to ensure the safety and well-being of clients experiencing acute emotional distress. Actively participate in debriefing sessions with the team to share experiences and enhance crisis response strategies.
Collaboration and Communication:
Foster effective communication and collaboration within the multi-disciplinary team. Represent on behalf of the team in meetings with external partners, stakeholders, and community organizations. Ensure that clients and their support persons are educated about mental health conditions, coping strategies, and available resources.
Administrative Responsibilities :
Assist with the development and implementation of policies and procedures related to service coordination. As needed, participate in interviews and onboarding of new team members, ensuring they are well-integrated into the team and trained in organizational practices. Provide regular reports to senior management on team performance, service delivery outcomes, and areas for improvement.
Documentation :
Per El Hogar and Sacramento County regulatory documentation policies & procedures, maintain accurate and timely documentation of client assessments, progress notes, treatment plans, and other relevant records in compliance with regulatory and organizational requirements (e.g., progress notes, adverse incident reports, discharge summaries, Medi-Cal billing, EHR forms, HMIS, etc.). Responsible for producing the required number of service units per month.
Quality Assurance :
Conduct regular chart audits and participate in internal and external utilization reviews to ensure adherence to quality standards and regulatory requirements. Implement and monitor quality improvement initiatives to enhance service delivery and client outcomes. Address any service delivery issues promptly, developing and implementing corrective action plans as needed.

Experience in mental health social services (Required); Work experience with adults who have mental health/substance abuse/homeless/ forensic behavioral population (Preferred).
