This role applies behavioral health principles to support lifestyle changes, reduce health risks, and provide consultation and treatment for mental health and psychosocial concerns. This position includes a focused effort on serving justice-impacted individuals , ensuring culturally responsive, trauma-informed care that addresses the unique needs of this population. Required Skills & Qualifications Education and Experience:
Master's Degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counseling Minimum of three (3) years of experience in a community mental health setting Experience working with multi-ethnic, underserved, and justice-impacted populations Bilingual in Spanish and English (preferred) Experience in a Managed Care settingLicensure/Certification:
California ACSW, AMFT, LCSW, or LMFT required Essential Duties and Responsibilities Clinical Patient Care Conduct assessments, evaluations, and provide mental health treatment Deliver individual and group therapy aligned with treatment plans Address psychosocial concerns impacting medical conditions Triage urgent and routine referrals from providers and staff Collaborate on intake scheduling and care coordination Participate in case conferencing with interdisciplinary teams Develop and implement client-centered treatment plans Ensure linkage to supportive services (case management, advocacy, medication, emergency services, etc.) Strong ability to engage and build trust with justice-impacted individuals using a trauma-informed, client-centered approach Experience with crisis intervention, de-escalation, and managing high-acuity behavioral health needs Strong assessment, documentation, and organizational skills, including proficiency with Electronic Health Records (EHR) and managing competing priorities Communication and Team Collaboration Present cases to multidisciplinary teams as needed Provide timely feedback to referring providers and staff Participate in Integrated Behavioral Health and staff meetings Engage in regular clinical and administrative supervision Administrative Responsibilities Maintain active professional licensure Ensure compliance with all documentation and medical record requirements Meet or exceed productivity standards Complete timely and accurate EHR documentation (notes, billing, authorizations) Align work schedules with operational needs Support data collection and program evaluation efforts St. John's Community Health is an Equal Employment Opportunity EmployerSimilar jobs in San Bernardino, CA
