Required Qualifications:
Experience working with youth, young adults, and families from diverse cultural, socioeconomic, and lived-experience backgrounds. Knowledge of trauma-informed, strength-based, and culturally responsive practices. Education and Experience:

This clinic-based position is responsible for supporting the intake coordination at clinic sites with clients, families and the clinicians. While also maintaining a caseload of clients both in person and virtually. This includes providing assessment, individual and family counseling, crisis intervention and case management. Job Responsibilities Provide clinical case management services to Mental Health Programs clients in custody through Juvenile Probation. identified as part of the Secure Track program of San Mateo County. Services may include individual, group, and family counseling, skills coaching, community referrals, coordination of care, crisis intervention, psycho- education, communication with other service providers, and collaboration with juvenile justice systems, client advocacy, and caregiver education. Services are provided in custodial settings, client homes, schools, community locations, and other approved environments. Collaborate as a member of a multidisciplinary team (MDT), including Behavioral Health and Recovery Services (BHRS), Human Services Agency (HSA), Juvenile Probation Department (JPD), and community-based organizations to coordinate treatment planning, service delivery, and ongoing support for youth and families. Communicate effectively with clinical team and actively participate in individual and group clinical supervision (maintaining a necessary ratio of supervision to face to face client hours per BBS requirement). Inform mental health leadership team immediately of any client crises, possible reporting mandates, and challenging clinical issues. Send all reports and letters to management for approval before delivery. Check e-mail daily and respond within 1 business day. Maintain high quality, HIPAA compliant clinical charting and records using electronic health care records system. Provide and properly document a minimum average of 12 direct service units a week in Diversion Program. Type all service documentation within 24 working hours of service provision and enter into ECM database by end of day each Monday for the previous week. Finalization of reviewed documentation must be complete by end of day Monday in order to avoid duplicate revision. Work closely with the Juvenile Hall staff, BHRS Clinicians, Probation Officers, and Attorneys working with the youth served. Probation will assign clients and families to Case Manager to assess and refer to appropriate services. Become and remain CANS certified. Enter all initial CANS assessments in ECM within 7-14 days of initial meeting with Client and family for Diversion clients Track weekly units either by using spreadsheet or calendar/planner. Client and guardian signatures are to be completed no later than the second client contact. CANS assessments are completed at opening and at closing of case. Depending on length of client's involvement and progress in program, a CANS assessment may also be completed at 6 and 12 month marks. Have consent paperwork re-signed at each anniversary date by legal guardian or client over 18 years old. Employ strength-based approaches to all facets of working with clients and their families.
