Education/Experience:
Applicants must meet any one of the following education and experience requirements: HS degree with six (6) years of direct service experience in the Mental Health field, OR Associates degree with four (4) years of direct service experience in the Mental Health field, OR Bachelor's degree with two (2) years of experience working in the mental health field.
License or Certification:
Valid California Driver's License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.
NOTE:
This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles. How you will make a difference (job overview): This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
Division/Program Overview:
Focus on providing prevention and early intervention to individuals (10 to 25) who are at risk for experiencing early psychosis. Working as a team to provide quality and comprehensive care to individuals and their families. Sessions are in the community, clinic, home and some telehealth.
