• + Provides referrals for patients within the agency and community.

+ Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care. + Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns. + Participates in multidisciplinary treatment team meetings, providing clinical input. + Formulates discharge plans in collaboration with the multidisciplinary treatment team, the patient, and the family, ensuring outpatient appointments are scheduled upon inpatient discharge.
