Job Description:

Position Overview Ultimate Staffing is actively seeking a dedicated, fully-bilingual (Spanish/English) Health Case Manager to join a mission-driven healthcare organization in Greeley, CO. This role provides intensive, client-centered case management to individuals with complex medical, behavioral health, and social needs. This position is ideal for a professional who is passionate about improving health outcomes for underserved and medically vulnerable populations and who values relationship-based, high-impact work in a clinical setting. Responsibilities Assess, evaluate, and reassess client needs to develop and maintain individualized care or wellness plans. Provide ongoing medical case management support to clients navigating chronic health conditions and life-stability challenges. Coordinate care with healthcare providers, behavioral health professionals, housing programs, and community agencies. Support clients with medical engagement, treatment adherence, benefits access, and basic needs coordination. Advocate for client needs while empowering individuals to actively participate in their care plans. Monitor client progress and adjust care strategies in collaboration with multidisciplinary teams. Maintain thorough, accurate, and timely documentation in electronic health and case management systems. Provide education, guidance, and emotional support to clients in a respectful, trauma-informed manner. Participate in case reviews, care conferences, and continuous improvement activities. Ensure all services align with healthcare regulations, program guidelines, and organizational standards.

Qualifications:

Bachelor's degree in Nursing, Human Services, Social Work, or related field required. Bilingual Spanish/English required. Registered Nurse (RN) license or relevant certification preferred. 2+ years of experience in case management or a related healthcare role. Strong interpersonal and communication skills. Proficiency in electronic health record (EHR) systems. Ability to work independently and as part of a team. Detail-oriented with excellent problem-solving abilities. Understanding of healthcare regulations and patient rights.
