Healthcare Professionals accepted) Licensed Clinical Social Worker (LCSW) - A Community Health Center that provides an array of primary care, women's healthcare, elder care and behavioral health services is looking for an experience LCSW to join their behavioral healthcare team. The LCSW will engage individuals of all ages in multidisciplinary assessments and psychiatric consultations to provide comprehensive psychiatric evaluation and treatment recommendations. Support integrated care with Psychiatry and Primary Care. Connect clients to appropriate services after completion of assessment. Provide clinical services for individuals, children and families, couples or groups of all ages regarding behavioral, substance abuse, psychiatric or emotional issues through the development and implementation of clinical assessments, treatment plans and discharge plans. Provide client centered clinical assessment and treatment care consistent with quality standards of care including evidence-based practices that result in improved health outcomes for clients. Work in collaboration with Primary Care Social Workers to support Psychiatric Consultation from Primary Care. Engage children and families in psychiatric assessment and connection to appropriate services. Will carry a reduced caseload of treatment clients.
Qualifications:
Master's in Social Work (MSW), 3-5 years' experience in related field, CT State Licensure (LCSW required)
