Schedule:
Day shift | Monday - Friday, 40 hours per week
Pay:
$32/hr.
Overview:
Seeking a Behavioral Health Case Manager to provide case management services in an outpatient behavioral health setting. In this role, you will support individuals with developmental disabilities, mental illness, or emotional disturbances by coordinating person-centered care, developing treatment plans, conducting assessments, and connecting clients with appropriate community resources. Key Responsibilities Conduct behavioral health assessments and develop individualized treatment plans. Provide person-centered case management services and ongoing care coordination. Monitor client progress, safety, and risk while ensuring appropriate interventions. Coordinate referrals to community resources and social service agencies. Collaborate with multidisciplinary teams and external providers to ensure continuity of care. Maintain accurate documentation and case records in accordance with organizational and regulatory standards. Provide consultation and support to internal staff and external agencies as needed. Promote client independence, recovery, and access to appropriate behavioral health services. Qualifications Master's degree in Behavioral Science, Social Work, Psychology, Counseling, or a related field 3+ years of case management experience, including assessment, care planning, service coordination, monitoring, and evaluation using effective interviewing and counseling techniques 3+ years of experience providing clinical treatment, counseling, or determining eligibility for behavioral health or human services as part of a multidisciplinary treatment team. #Workwolf
