A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.
Performs behavioral health case management (Targeted/Community and Forensic) functions of assessment, treatment/service plan development, plan implementation and review, yearly in-depth assessments and ongoing status assessments, links clients with appropriate services and resources (e.g., social security benefits, food stamps, medical insurance, medical care, education, behavioral health care services, support groups, etc.) advocates for acquisition of services and resources necessary to implement the plan, coordinates the delivery of services in the plan with the help of the client and with client consent, client's family/natural support system, monitors service delivery to evaluate client's progress. For forensic clients develops, monitors, and amends clients' Conditional Release Plans and files affidavits, when clients violate their conditional release plans. Writes court reports and attends court hearings. Completes residential utilization management applications/referrals with required documentation for submission to BBHC. In addition, completes assessments and documentation required for the acquisition of services and resources required by the system of care. Maintains excellent time management and organization skills to accomplish a large volume of work timely which includes completing all documentation required by funders.
REQUIREMENTS
1. Have a bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of one year of experience working with adults experiencing a serious mental illness. or have a bachelor's degree from an accredited university or college and three years full time or equivalent experience working with adults experiencing a serious mental illness. 2. Must possess a valid Florida Driver's License and have dependable transportation. 3. Required to travel throughout Broward County for home visits and appointments. There may be some travel outside of Broward County. 4. Must be able to use the telephone, computer, copy/fax machine, and any other needed office equipment and agency vans. 5. Familiarity with mental health and substance abuse system in Broward County. 6. Required to acquire and maintain jail clearance. 7. Required to maintain a caseload and cover other caseloads, as needed. 8. Must be medically and lawfully able to drive a company vehicle as needed. 9. Must have excellent organizational, time management, and writing skills. 10. Have strong interpersonal and communication skills. 11. Prefer experience working with the Broward County court system. 12. Prefer knowledge of the effects of substance abuse and mental illness, knowledge of special populations (medical issues, conditional release, personality disorders, co-occurring disorders. 13. Must apply and obtain certification or be a Certified Behavioral Health Case Manager with the Florida Certification Board at the time of hire. 14. Have knowledge of available resource in the community for adults with a serious mental illness. 15. Is knowledgeable of compliance with state and federal statutes, rules, and policies that affect the target population.
