Our accredited FACT Team works collaboratively to provide wrap around, community based behavioral health services using a multi-disciplinary team approach. Provide integrated treatment for co-occurring mental illness and substance use disorders for individuals with a history of substance use. Complete substance use assessments that consider the relationship between substance use and mental health. Assess and track participants' stages of change readiness and treatment.
