Job Description
The Advocate Program, Inc. is a private, not-for-profit, 501 c (3) justice agency that offers probation, diversion, community service, DUI and traffic programs, domestic violence programs, substance abuse and mental health evaluations and treatment to court-ordered individuals. It has expanded to work with juveniles and families in the juvenile justice field, families impacted by family conflict and violence, and homeless veterans and their families.
SUMMARY:
TheMENTAL HEALTH
Care Coordinator manages direct service staff that manage a client caseload to manage service integration, treatment planning coordination, transitional planning, and linkages. Education and/orExperience:
Bachelor's degree in nursing, Social Work, Healthcare Administration, or a related field. 1-2 years of experience in care coordination, case management, or related field. Bilingual inEnglish & Spanish Qualification Requirements:
Strong understanding of case management/care coordination principles and recovery-oriented system of care principles. Strong organizational skills with the ability to manage multiple client cases simultaneously. Excellent communication and interpersonal skills to work with diverse client populations and healthcare teams. Proficiency in electronic health record systems and basic computer skills. Knowledge of community resources, insurance processes, and healthcare regulations. Empathy, patience, and a passion for supporting clients through their care journey.Responsibilities:
Conduct initial assessments of client needs, goals, and barriers to accessing care. Develop individualized care plans in collaboration with service providers, clients, natural supports and families. Update care plans regularly based on changes in client condition or treatment needs. Coordinate appointments, referrals, and follow-up care to ensure clients receive timely services. Communicate with providers, specialists, and other healthcare services to facilitate smooth transitions in care. Monitor and track client progress, following up to address any issues with appointments, treatments, or access to services. Serve as a central point of contact for patients and their families, offering guidance and support in navigating the continuum of care. Advocate for clients' needs, helping them understand their treatment options, care plans, and resources available. Maintain accurate records of client interactions, care plans, and coordination activities Ensure all documentation meets regulatory and compliance standards.
