Job Description Insight Global is looking for a FACT Team Lead/Manager at a large behavioral healthcare organization. This individual will lead community-based behavioral health initiatives, conducting member assessments, developing individualized care plans, and connecting clients to primary care, behavioral health, specialty services, and community resources. The role combines leadership and direct clinical support, overseeing member progress, collaborating with multidisciplinary care teams, participating in community visits, maintaining a reduced caseload, and ensuring program compliance through reporting, data management, and support of client independence. - 2+ years of experience
- Master's degree from an accredited university with major course work in the areas of health related/psychology/social work areas.
- Active LCSW, LMFT or LMHC license in State of Florida require.
- Must possess a valid Florida driver's license and have acceptable driving record.
- Five years working with individuals dealing with Severe and Persistent Mental Illness and or co-occurring mental health and substance use disorders.
- Three years' experience in Mental health managerial positions strongly preferred.
