Job Overview Join our dynamic team where you can utilize your degree to make a meaningful impact while enjoying your work! If you are ready for a change where you can make a difference in someone's life, this is the place for you. We are looking for two professionals to join our program. We are seeking a Professional to be a Case Manager within our supported living program. Case Managers oversee a small caseload of Adults With Developmental Disabilities. You will oversee the clients goals such as improving social skills, budgeting, or cooking healthy meals. As the Case Manager you will schedule, and attend important medical appointments to support the client. Being able to communicate what is going on with the team is vital for their success. Regularly meeting with the client to discuss things such as food access (food banks/food stamps), health/weight/exercise, hygiene, house cleanliness, communication, job searches, family relationships, community access, etc. assist with the clients ability to be as independent as possible. Case Managers often deal with situations in real time such as roommate or staffing conflicts. Being able to help the clients navigate disagreements or give them tools to handle problems is vital for their participation in this program. If you are detail oriented, patient, nurturing, strong communication skills, enjoy teamwork, are good at paperwork, and have a passion to help others, this is the job for you. You MUST have a 4 year degree in a health or human service field such as psychology, sociology, social work, education, health, criminal justice, etc. to apply for this job as well as one year experience working with people with Developmental Disabilities to be the Case Manager.
