We are seeking compassionate and motivated individuals to join our team as Case Managers and Community-Based Rehabilitation Specialists (CBRS). In this dual-role position, you will provide skill-building services in the community and support clients in accessing essential resources to promote mental wellness and self-sufficiency.Key Responsibilities:
Community-Based Rehabilitation (CBRS): Work one-on-one with clients in the community to promote independent living skills. Assist clients with setting and working toward recovery-based goals. Provide support with tasks such as budgeting, social skills, scheduling, and coping strategies. Maintain detailed, timely documentation for all services provided.Case Management:
Link clients to community resources including housing, employment, transportation, and medical/psychiatric care. Advocate on behalf of clients to ensure access to appropriate services. Collaborate with treatment teams to coordinate client care plans. Monitor and evaluate client progress and adjust services as needed.Requirements:
Bachelor's degree in Psychology, Social Work, or a related human services field (Required) Minimum 1 year of experience working in a behavioral health or mental health setting Strong written and verbal communication skills Valid driver's license, reliable transportation, and proof of insurance Ability to pass a background check Comfortable working independently in the community and in client homesPreferred Qualifications:
CBRS certification (or eligible for certification in Idaho) Knowledge of Medicaid documentation standards Familiarity with electronic health records (EHR) systemsBenefits:
