Onsite Case Manager Deborah's Place - 4.3 Chicago, IL Job Details Full-time $48,000 - $50,000 a year 1 day ago Qualifications Mental health and behavioral care Driver's License Bachelor's degree Case management Conflict management Productivity software Crisis intervention counseling Full Job Description Position Summary This position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals. Essential Duties and Responsibilities Case Management Services Provide case management services including intakes, assessments, service planning, staffing, individual counseling, referral, and advocacy. Provide crisis intervention in mental health and/or medical emergencies. Provide on-call assistance with after hour and weekend emergencies. Assist with applications and advocacy related to public benefits. Plan and implement therapeutic group activities. Work with tenants to achieve their goals related to housing, income and health. Regularly assess and respond to tenants' motivation and readiness using motivational interviewing skills. Engage in clinical supervision, in both group and individual formats. Work with tenants to address issues in order to prevent eviction. Record Keeping and Data Quality Prepare and maintain current tenant files. Enter all services and required information into HMIS and other service and outcome tracking databases as required. Work collaboratively with the Program Data Manager to ensure data quality. Other Maintain required professional knowledge and job skills. Attend and participate in staff development opportunities. Represent Deborah's Place mission and values to participants, visitors and others. Perform other duties as assigned.
Qualifications and Requirements Education:
Bachelor's Degree in Social Work or related field. Master's degree preferred. Licensure and/orCertification:
LCSW preferredJob Experience:
2-3 years of case management experienceSpecial Knowledge & Skills:
Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, good oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and must have own vehicle.Similar jobs in Chicago, IL
