Overview We are seeking a dedicated and compassionate Wraparound Coordinator to join our multidisciplinary team committed to providing comprehensive, individualized care for children and families facing complex challenges. The Wraparound Coordinator plays a vital role in facilitating collaborative planning, coordinating community resources, and ensuring that each client receives tailored support to promote stability, health, and well-being. Responsibilities Lead the development and implementation of individualized care plans through family-driven, strengths-based approaches. Coordinate services across healthcare providers, social services, educational institutions, and community organizations to ensure seamless support for clients. Conduct comprehensive assessments of patients' medical, behavioral health, and social needs utilizing clinical judgment and relevant medical terminology. Facilitate intake processes, discharge planning, and ongoing case management to promote continuity of care. Maintain accurate documentation within electronic health records (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality. Collaborate with families and caregivers to foster engagement, provide education, and support decision-making processes. Prioritize tasks effectively in high-pressure environments such as hospital settings—including Level I Trauma Centers, ICU units, or pediatric wards—and manage acute care cases efficiently. Utilize clinical case management skills to monitor progress, adjust plans as needed, and advocate for client access to community resources. Engage in utilization management activities by evaluating service appropriateness and supporting discharge planning efforts. Demonstrate proficiency in medical terminology and physiology knowledge relevant to inpatient and outpatient settings. Work closely with multidisciplinary teams including behavioral health specialists, addiction counselors, mental health professionals, and social workers. Qualifications Bachelor's degree in social work, nursing, psychology, or a related healthcare field; advanced degrees preferred. Minimum of two years' experience working in hospital environments such as ICU, emergency medicine, or pediatric units; experience within Level II or Level I Trauma Centers highly desirable. Proven background in hospital medicine, acute care settings, outpatient clinics, or post-acute care environments. Strong familiarity with EMR/EHR systems and medical records management. Certification or training in Basic Life Support (BLS), CPR, and relevant clinical skills. Experience working with children and individuals with disabilities or mental health conditions from diverse cultural backgrounds. Knowledge of managed care processes and utilization management principles. Excellent patient service skills combined with the ability to communicate complex information clearly to clients and families. Competence in task prioritization under demanding circumstances while maintaining attention to detail. Ability to work collaboratively within multidisciplinary teams focused on behavioral health, diagnostic evaluation, case management, and community resource coordination.
