Job Overview We are seeking a dedicated and compassionate Social Service Coordinator to join our dynamic team. Responsibilities Assess client needs through active listening and thorough case evaluations, developing personalized service plans. Coordinate with social service agencies, healthcare providers, housing programs (including Section 8), Medicaid, Medicare, and workforce development initiatives to secure resources for clients. Provide crisis intervention and crisis management support during emergencies or urgent situations, ensuring client safety and well-being. Deliver addiction counseling and behavioral health support, utilizing motivational interviewing techniques to promote positive behavioral changes. Assist clients with working with seniors, individuals with disabilities, or those involved in child welfare or childcare programs by connecting them with appropriate services. Maintain accurate documentation of client interactions, service plans, and progress notes while demonstrating strong writing skills. Supervise and mentor junior staff or volunteers when applicable, fostering a team-oriented environment focused on client empowerment. Skills Proven experience in social work or social services with a focus on case management and client communication. Expertise in crisis intervention, crisis management, behavioral health support, and addiction counseling. Strong knowledge of Medicaid, Medicare, Section 8 housing programs, child welfare policies, and working with geriatric patients. Ability to work effectively with culturally diverse communities and individuals with disabilities through culturally sensitive approaches. Bilingual skills are highly desirable to serve a broader range of clients effectively. Certifications in CPR and First Aid are preferred to ensure safety during emergency situations. Excellent organizational skills combined with the ability to multitask efficiently while maintaining attention to detail. Join us in making a difference!
