Job Description

Job Overview:
The Case Manager is responsible for coordinating comprehensive care and discharge planning services for clients in an inpatient behavioral health and detox setting. This role ensures continuity of care through collaboration with clinical, medical, and administrative teams, as well as outside providers, families, and community resources.
Duties & Responsibilities:
  • Coordinate care for clients including referrals to outpatient programs, sober living environments, community mental health providers, and social service agencies.
  • Facilitate communication between clients, families, treatment teams, and external agencies to ensure smooth transitions of care.
  • Maintain accurate, timely, and confidential documentation in accordance with facility policies and state regulations.
  • Educate clients and families about available treatment options, resources, and recovery supports.
  • Advocate for client needs and ensure that services provided align with individual treatment goals.
  • Attend daily treatment team meetings to review progress and update plans as appropriate.
  • Assist with insurance authorizations, utilization reviews, and documentation necessary for continued stay and discharge.
  • Support clients in developing relapse prevention and aftercare plans to maintain long-term recovery.
  • Uphold facility standards of ethical and trauma-informed care.
Requirements:
    Education:
    Bachelor's degree in Social Work, Psychology, Counseling, or a related behavioral health field required. Master's degree preferred.
      Licensure/Certification:
      LSW, LCSW, LMHC, LMFT, or related Indiana licensure preferred.
        Experience:
        Minimum of 1-2 years of experience in case management, mental health, or substance use disorder treatment required; inpatient or residential experience strongly preferred.
        Pay:
        $53,000.00 - $55,000.00 per year
