Job Description

SCMHCC is seeking a compassionate and driven Crisis Case Manager to deliver intensive, goal-directed behavioral health services to children and adults experiencing elevated mental health symptoms. Working within our Crisis Department, you will provide stabilization support, mobile crisis response, and community-based care coordination — helping individuals remain safely in the least restrictive environment possible. The candidate must be able to work a regular schedule to 7PM and also is required to be on call for regular intervals.
WHAT YOU'LL DO
Provide in-person and phone crisis stabilization support to consumers, families, and community members at SCMHCC and off-site locations. Deliver intensive, medically necessary crisis case management services to children and adults experiencing acute mental health episodes. Collaborate with caregivers, family members, treatment providers, and community partners to develop and implement effective crisis support plans. Provide timely, accurate updates on consumer status, response to intervention, and follow-up planning. Support CSS/CBS high-needs consumers and assist assigned team members during periods of elevated caseload. Attend CSS/CBS staffing and group supervision meetings to build familiarity with high-needs clients. Build and maintain relationships with schools, juvenile justice, courts, law enforcement, and hospitals to create a supportive community network for consumers. Assist consumers in identifying and accessing natural, community-based resources that support daily functioning and independence. Ensure compliance with all deadlines, contractual obligations, and licensing requirements related to crisis services. Participate in on-call rotation and respond to mobile crisis requests as needed. Attend required SCMHCC training sessions and proactively identify additional learning opportunities to strengthen crisis competency. Support continuous improvement of the Crisis Department through collaboration and coordination. Maintain regular and predictable attendance — an essential function of this role.
PHYSICAL REQUIREMENTS

MOBILITY
Ability to travel to and mobilize within client locations including homes, schools, hospitals, correctional facilities, and other community sites. VISION Sufficient visual acuity to observe client behavior, body language, and environmental safety conditions during in-person interactions.
HEARING & SPEECH
Ability to hear and clearly communicate with clients, families, and team members in person and by telephone, including in environments with background noise.
PHYSICAL ACTIVITY
Ability to sit, stand, walk, and drive for extended periods. Occasional lifting or carrying of materials up to 25 lbs may be required.
ENVIRONMENT
Ability to work in varied community settings including client homes, outdoor environments, and facilities that may present unpredictable conditions.
STRESS TOLERANCE
Ability to perform effectively in high-stress, fast-paced crisis situations while maintaining professional composure and sound judgment.
TECHNOLOGY
Ability to use a computer, smartphone, and electronic health record systems to complete documentation and communicate with team members. DRIVING Ability to operate a motor vehicle safely to travel between agency sites and community locations as required by the role.
