McPherson, KS Schedule:
Full-Time Make a real difference in the lives of children, adolescents, and families. Prairie View is seeking a compassionate, motivated School-Based Community Case Manager to provide community-based behavioral health services to youth and families. This role focuses on strengths-based support, care coordination, crisis intervention, and advocacy within school and community settings. Key Responsibilities Provide community-based case management services in school settings based on treatment plans from qualified mental health providers Support children, adolescents, and families through strengths-based interventions, crisis response, and problem-solving Coordinate care with schools, families, referral sources, and community partners Link families to community resources and social services Assist with goal-setting, life skills, parenting skills, and advocacy Maintain accurate, timely clinical documentation and meet billing expectations Participate in treatment planning, supervision, and professional development Provide community education and outreach as needed Transport clients when required Qualifications Bachelor's degree in behavioral health or a related field (or equivalent) At least 21 years of age Experience or demonstrated skill in case management Strong communication, documentation, and interpersonal skills Ability to work effectively with children, adolescents, and families Knowledge of child/adolescent development and behavioral health Ability to manage boundaries, safety, and crisis situations Valid Kansas Driver's License and insurable driving record Ability to pass background screenings (KBI, DCF, Central Registry) Ability to meet physical activity requirements, including participation in restraint training Basic computer and documentation skills Kansas licensure and continuing education (as applicable) What We Offer Meaningful, mission-driven work Ongoing training and professional development Supportive supervision and collaborative team environment Opportunities to grow clinical and leadership skills Access to Relias Learning and continuing education resources

$24.15 per hour
