Job description We are looking for an experienced Case Manager to join our team. The successful candidate will be responsible for providing case management services to clients, including assessment, planning, implementation, monitoring, and evaluation of services. The Case Manager will also provide advocacy and support to clients, as well as coordinate with other professionals to ensure the best possible outcomes. The ideal candidate will have excellent communication and organizational skills, as well as a strong commitment to helping those in need.
