Coordinate inpatient and outpatient services, ensuring smooth transitions of care across settings such as Level I Trauma Centers, PICUs (Pediatric Intensive Care Units), and community clinics. Collaborate with multidisciplinary teams to facilitate diagnostic evaluations, behavioral health interventions, addiction counseling, and mental health support tailored to each patient's unique needs. Manage electronic health records (EHR) efficiently while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Engage with patients' families and caregivers to provide education, emotional support, and discharge planning that aligns with their cultural backgrounds and individual preferences. Connect patients with community resources related to social work, managed care, disability services, and behavioral health programs to promote holistic recovery. Qualifications Proven hospital experience with exposure to Level II or Level I Trauma Centers; ICU or PICU experience is highly desirable. Strong background in acute care settings such as emergency medicine, inpatient units, or outpatient clinics. Experience working with children, individuals with disabilities or substance use disorders; familiarity with mental health counseling is a plus. Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management principles. Proficiency in EMR/EHR systems and electronic health records management; understanding of HIPAA compliance is essential. Excellent client communication skills with the ability to work effectively with diverse cultural backgrounds and language needs; bilingual fluency is required.
