Job Description

This position encompasses the following duties:

Advocate for patient needs by conducting ongoing psychosocial needs assessments and communicating these needs to the treatment teamPlan and implement appropriate inpatient disposition in collaboration with the treatment teamEffectively identify patient, family, institutional and community barriers to discharge or transitions in treatment. Coordinate referrals and facilitate transitions in treatment while addressing the barriers identifiedCoordinate the development and formulation of psychosocial and case management needs assessments through available resources in the communityFacilitate and/or provide direct clinical therapeutic interventions to prevent relapse and ensure continuity of care based on physician care plansProvide information/education to patients and their families/significant others regarding illness, recovery and treatment optionsWork collaboratively and professionally with the treatment team in maintaining a collaborative relationship with the outside providers and the reimbursement team to optimize the provision of high-quality patient care
Qualifications:
Master's Degree in social work required with a current valid LICSWBilingual (Spanish speaking) required.

Exhibits ability to be competent in the following areas:

Individual case management and knowledge of managed careDe-escalation/restraint management techniquesMotivational Interviewing and short-term treatment techniquesStrong ability to assess and evaluate patient needs and develop treatment plans to address the identified needs.

Strong organizational skillsAbility to be flexible in a fast-paced work environmentBrief family interventions as required

Pay Range:
$41.00 - $53.00The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
