Position Summary The Adult Mental Health Targeted Case Manager (TCM) provides culturally appropriate support for adults with Serious and Persistent Mental Illness (SPMI). The primary goal is to help clients access needed medical, social, educational, vocational, and other services related to their mental health needs while addressing all social determinants of the client's health. This community-based role requires the Case Manager to meet clients in their homes and community settings to promote stability and independence. 2. Essential Duties and Responsibilities Conduct comprehensive functional assessments when meeting the clients and their support, and update them semi-annually. Develop a collaborative individual community support plan with the client and their support network, and update it every 180 days to reflect current goals and needs. In a person-centered approach, refer the client to community resources and natural supports identified in the assessment and support plan to meet their needs. Contact the client monthly to review their progress towards support plan goals and address any concerns or barriers. Partner with clients to identify strengths, cultural values, and self-identified goals to promote recovery and community integration. Connect clients to crisis intervention resources within 24 hours of a reported crisis. Accept and process all county referrals, including those for individuals under civil commitment. Serve as an advocate for clients to ensure they receive the benefits and services for which they are eligible. Maintain accurate and timely documentation of all client contacts, assessments, and plans in the designated Electronic Health Record (EHR) system. Adhere strictly to the Minnesota Government Data Practices Act (MGDPA), HIPAA, and the Minnesota Health Records Act. Provide every client with a "Notice of Privacy Practices" and "Tennessen Warning" at initiation. Record client-related activities (assessment, ICSP development, referral/linkage, monitoring) accurately. Ensure invoices are submitted within required timeframes. 3. Qualifications and Requirements Minimum Qualifications Must meet one of the following criteria: A bachelor's degree in one of the behavioral sciences or related fields (psychology, social work, nursing, etc.) from an accredited college or university, AND at least 2,000 hours of supervised experience in the delivery of services to adults with mental illness; OR A graduate degree in one of the behavioral sciences or related fields; OR Is a registered nurse without a bachelor's degree, but who has at least three years of experience in the delivery of services to adults with mental illness. Skills and Competencies Must adhere to the company's professional boundaries and clinical ethics policies, including the strict prohibition of dual relationships. Ability to use company-issued mobile devices and navigate the EHR platform's user interface. Must have a valid driver's license, reliable vehicle, and maintain required insurance levels for community travel. 5.
Work Environment and Physical Demands Hybrid Work Structure:
This position involves both office-based administrative work and community-based client visits. The company reserves the right to adjust work arrangements based on performance and/or business needs.
