Job Description

Job Overview Determines clinical interventions for MI and or SUD issues, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary team and all other contacts. Functions as an interdisciplinary team member. Accepts supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice.
Schedule:
2 shift per schedule period OR equivalent of 4 on-call shifts

Demonstrates clinical competence in the treatment planning process. Facilitates treatment planning and oversee management of each case. Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team. Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs. Implements, reassesses and adjust treatment plan as necessary. Demonstrates clinical competence to assess individual patient needs. Completes an interdisciplinary assessment tool with each patient. Updates interdisciplinary assessments each week or per regulatory requirement. Demonstrates clinical competence in disposition planning. Interfaces with government agencies such as court, ombudsmen, county services Completes referral process for next level of care Communicates to other providers in the community Sets up intake appointments Completes needed paperwork to facilitate this process i.e. applications, letters of recommendation, etc. Required Qualifications Master of Science in psychology, social work, nursing, or closely related field. 1 year supervised experience in mental health and chemical dependency including comprehensive assessment, individual, group, and family therapy. Licensed Alcohol and Drug Counselor (LADC) Licensed Graduate Social Worker (LGSW) or eligible for independent clinical licensure as a LMFT or LPCC Preferred Qualifications 2 years experience in mental health and chemical dependency.
