In this vital role, you will provide comprehensive mental health assessments, develop individualized treatment plans, and deliver evidence-based interventions to diverse patient populations across inpatient, outpatient, and post-acute care settings. Your expertise will support patients navigating complex behavioral health challenges, ensuring they receive compassionate, timely, and effective care. Responsibilities Conduct thorough patient assessments to evaluate mental health status, physiological factors, and social determinants impacting well-being. Develop and implement personalized treatment plans aligned with best practices in behavioral health, trauma care, and addiction counseling. Collaborate with multidisciplinary teams including physicians, social workers, nurses, and case managers to coordinate patient care and discharge planning. Manage intake processes for new patients, ensuring accurate documentation in medical records while adhering to HIPAA regulations for patient privacy and confidentiality. Participate in diagnostic evaluations and utilization management to optimize patient outcomes and resource utilization within hospital medicine and outpatient clinics. Provide crisis intervention and support for patients experiencing acute psychiatric episodes or trauma-related distress in Level I or Level II trauma centers. Engage in case management activities such as post-acute care planning, outpatient follow-up coordination, and outpatient clinic services for diverse populations including pediatrics and adults. Skills Extensive hospital experience with a strong background in acute care settings such as ICU, emergency medicine, or trauma centers. Proven expertise in behavioral health services including mental health counseling, addiction counseling, and social work practices. Familiarity with medical terminology, physiology knowledge, diagnostic evaluation techniques, and patient assessment protocols. Experience working within managed care environments with skills in discharge planning, utilization management, and medical records documentation. Ability to effectively handle intake procedures and conduct comprehensive case management for inpatient and outpatient populations. Knowledge of HIPAA regulations combined with excellent patient service skills to ensure confidentiality while delivering compassionate care. Competence in handling complex cases involving pediatrics or post-acute care transitions within hospital or clinic settings.
