Job Description

Job Summary We are seeking a dedicated and energetic Qualified Professional to join our Community Support Team (CST). In this vital role, you will provide comprehensive support and case management services to individuals with complex behavioral health, medical, and social needs within the community. Your expertise will help coordinate care, facilitate access to services, and promote recovery and stability for diverse populations. This position offers an exciting opportunity to make a meaningful impact by working collaboratively with healthcare providers, social services, and community organizations to improve patient outcomes. Responsibilities Conduct thorough patient assessments, including medical history review, physiological evaluation, and behavioral health screening to determine individual needs. Develop personalized care plans that incorporate discharge planning, outpatient services, post-acute care options, and community resources. Coordinate inpatient and outpatient care transitions, ensuring seamless communication between hospital teams and community providers. Manage utilization review processes such as intake evaluations, diagnostic assessments, case management, and discharge planning to optimize patient care pathways. Maintain accurate medical records in compliance with HIPAA regulations while documenting patient interactions, assessments, and treatment plans. Engage in patient service activities including intake procedures, behavioral health counseling, addiction counseling, and mental health support tailored to pediatric or adult populations. Collaborate with multidisciplinary teams in hospital settings such as Level I Trauma Centers or Level II trauma centers, ICU units, PICUs (Pediatric Intensive Care Units), emergency departments, or clinics specializing in hospital medicine or behavioral health. Skills Extensive hospital experience with familiarity in acute care environments such as ICU settings or emergency medicine units. Proven expertise in managing complex cases within Level I Trauma Centers or similar high-acuity facilities. Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and patient assessment protocols. Experience working in inpatient or outpatient settings across various specialties including pediatrics, post-acute care, or managed care environments. Proficiency in discharge planning, utilization management, medical records documentation, and intake procedures. Background in behavioral health counseling, mental health therapy, addiction counseling or social work is highly desirable. Ability to navigate HIPAA regulations diligently while handling sensitive patient information; excellent communication skills for effective collaboration with healthcare teams and community agencies.
