Mental Health Case Manager Per Diem About the Company Healthcare Commons, Inc. is a not-for-profit, licensed community mental health agency that has served Salem County for more than 70 years, as well as parts of Cumberland and Gloucester counties. Across multiple locations, we provide comprehensive behavioral health and support services for children, adolescents, and adults—including outpatient counseling, residential programs, partial care, crisis services, early intervention, case management, and family support. We are conveniently located near Delaware, Pennsylvania, and Southern New Jersey. About the Role The Mental Health Case Manager Per Diem provides case-management services in both on-site and community settings. This role ensures coordinated service delivery, supports clients with accessing appropriate programs and resources, and assists individuals in achieving stability and improved quality of life. This is a flexible per diem position ideal for candidates looking to contribute meaningful support within a mission-driven environment. Key Responsibilities Provide case-management services in both on-site and off-site environments Ensure coordinated service delivery and appropriate linkage to programs, services, and facilities Assist clients in navigating behavioral health and community resources Maintain accurate, timely documentation in accordance with agency and licensing standards Participate in meetings, trainings, and interdisciplinary collaboration Perform additional related duties as assigned Qualifications Bachelor's degree in a mental health-related discipline required Minimum of 1 year of post-degree mental health case management experience required Excellent written and verbal communication skills Strong organizational, interpersonal, and problem-solving abilities Valid driver's license in good standing required Bilingual (Spanish) preferred
Compensation & Schedule Pay:
$25.00 per hour
