Mental Health Case Manager - Full Time About the Company Healthcare Commons, Inc. is a not-for-profit, licensed community mental health agency that has served Salem County for more than 70 years, as well as parts of Cumberland and Gloucester counties. Across multiple locations, we provide comprehensive behavioral health and support services for children, adolescents, and adults—including outpatient counseling, residential services, partial care, crisis intervention, early intervention, case management, and family support. We are conveniently located near Delaware, Pennsylvania, and Southern New Jersey. About the Role This full-time position provides case-management services in both on-site and community settings. The role supports coordinated service delivery, helps clients access appropriate programs and resources, and ensures continuity of care. Key Responsibilities Provide case-management services in on-site and off-site environments Ensure coordinated service delivery and successful client linkage with programs, facilities, and community resources Assist clients with navigating behavioral health and social support systems Maintain accurate and timely documentation in compliance with agency and state standards Participate in team meetings and interdisciplinary collaboration Perform other related duties as assigned Qualifications Bachelor's degree in a mental health-related discipline required Minimum of 1 year of post-degree mental health case management experience required Excellent written and verbal communication skills Strong organizational, interpersonal, and problem-solving abilities Valid driver's license in good standing required Bilingual (Spanish) preferred
Compensation & Schedule Pay:
$21.64 per hour (approximately $45,000 annually )
