• LCSW Specialty - Center for Healthy Aging - Rhinebeck, NY The LCSW provides integrated behavioral health services, delivering assessments, brief therapeutic interventions, and care coordination for geriatric patients.

This role supports improved psychosocial functioning,facilitatesaccess toappropriate levelsof care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive,patient centeredcare. Key Responsibilities

• Conduct behavioral health assessments and develop individualized treatmentand transition plans.
• Provideevidence-basedinterventions (CBT, DBT, MI, SBIRT, mindfulness,trauma‑informedcare).
• Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation.
• Support behavioral health integration in primary care through collaboration, consultation, and care coordination.
• Facilitate referrals to higher levels of care and connect patients to community and hospital resources.
• Document all care accurately andtimelywhile adhering to regulatory and compliance standards.
• Participate in quality initiatives,maintainprofessional knowledge, and modelNorthwellHealth values. Qualifications
• Licensed Clinical Social Worker (LCSW) in CT or NY, depending on work location.
• Minimum 3-5 years of clinical experience.
• Experience in medical social work, crisis intervention, behavioral health, or integrated care preferred.
• Strong clinical judgment, diagnostic skills, communication abilities, andproficiencywithevidence-basedmodalities. Skills
• Expertiseindiagnostic assessment, treatment planning, and crisis management.
• Knowledge of mental health and substance use conditions, includingcooccurringdisorders.
• Strong communication, documentation, and organizational skills.
• Ability to collaborate effectively with medical providers, care teams, and community partners.
