Job Description

Overview Soul Saving Station Shelter is seeking a skilled, compassionate, and accountable Case Manager to provide direct support and guidance to residents as they work toward stability, independence, and long-term success. This role is ideal for someone who combines clinical understanding with real-world case management strength —able to build trust with clients while also setting clear expectations, managing progress, and navigating complex systems such as housing, healthcare, and social services. What You'll Do Provide one-on-one case management and supportive counseling using approaches such as motivational interviewing and behavioral health techniques Develop and implement individualized service plans that address housing, health, employment, and personal stability goals Guide residents through systems such as Section 8 housing, DSS services, Medicare, and workforce development programs Monitor resident progress, hold clients accountable to goals, and adjust plans as needed Conduct crisis intervention and respond to behavioral health or situational emergencies Collaborate with internal staff and external providers to ensure coordinated, holistic care Perform outreach and community-based engagement as needed (home visits, partner coordination) Support residents in overcoming addiction, mental health, and life barriers through consistent engagement and encouragement Maintain accurate, timely documentation of all services, interactions, and outcomes Assist in mentoring or supporting junior staff, if applicable What We're Looking For Experience in social work, case management, behavioral health, or a related field Strong understanding of addiction, mental health, and crisis intervention practices Familiarity with housing systems (Section 8), DSS, Medicare, and community resources Ability to balance compassion with accountability Excellent communication and motivational skills Strong documentation, organization, and follow-through Ability to manage high-stress situations and respond effectively in crisis scenarios Professional, emotionally grounded, and dependable Ideal Candidate You are someone who: Is passionate about helping people create real, lasting change Can build trust while still holding firm boundaries Is confident navigating complex systems and advocating for clients Thrives in a fast-paced, mission-driven environment Is both people-focused and results-oriented Can remain calm, decisive, and supportive during crises Why Join Us Meaningful work that transforms lives and restores hope Opportunity to walk closely with individuals on their journey to stability Collaborative, mission-driven team environment A role that blends compassion, structure, and impact
