Job Description

Unity House of Troy is seeking a compassionate individual for a full-time Case Manager position for the Crisis Residence. The schedule is Sunday-Thursday 2pm-10pm.

Position may be eligible for a sign on bonus.

Key ResponsibilitiesProvide daily crisis intervention and case management services to program participants Offer consistent encouragement, guidance, and positive reinforcement Assist participants in developing Wellness Recovery Action Plans (WRAP) and Safety Plans Provide accompaniment, advocacy, and emotional support as needed Support admissions and discharge planning processes Help individuals build and improve daily living and independent living skills Connect participants with community resources and referrals aligned with their goals Participate in a rotating on-call schedule Foster a supportive environment that promotes inclusion, respect, and belonging QualificationsValid, clean New York State drivers license required Must have reliable transportation High School Diploma or GED required (degree preferred but not required) In lieu of a degree, candidates must have:13 years of experience working with individuals with serious mental illness and/or substance use disorders, and/or Lived experience with mental health, behavioral health, or substance use challenges (or as a caregiver/parent of someone with these experiences) Skills & RequirementsAbility to work independently and collaboratively within a team Strong verbal and written communication skills Basic computer proficiency Relevant certifications (if applicable) must be provided
