Job Description
Overview We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping individuals in need and possess a strong background in behavioral health and crisis management. Responsibilities
- Provide case management services to clients, including assessment, planning, and implementation of care plans
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care for clients
- Conduct regular client evaluations to monitor progress and adjust care plans as needed
- Offer counseling and support to clients dealing with addiction or mental health issues
- Supervise and mentor clients in workforce development programs
- Assist with childcare arrangements for clients when necessary
- Oversee child welfare cases and ensure the safety and well-being of children involved
- Utilize motivational interviewing techniques to empower clients towards positive change Experience
- Previous experience in case management, particularly in areas such as Medicare, child welfare, addiction counseling, or workforce development
- Strong knowledge of crisis management strategies
- Ability to supervise and mentor individuals effectively
