Job Description

Position Summary:
Responsible for the full client care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients health and human services needs.
Position Summary:
Maintains caseloads and utilization, by maintaining regular contact with clients in community and office, to monitor and assess progress.
Meets with clients:
individually or in group to assess needs; collaborates with clients, families, other service providersDocuments all clinical activities. Documentation will meet specific program , funder and credentialing entity criteria .All documentation will be completed in a timely manner in order to meet program timelines.

Contact will be made at a minimum of every 90 days.

Documentation will support attempts to engage, re-engagewith clients on case load.

Delivers on-going services clients which may include but is not limited to : crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting assistance, ADL assistance and referral and linkage to community resources as needed.

Assist clients in more effectively functioning within or preparing for re-integration into the community.

May provide supportive counseling to clients when needed.

Provide sign-off for Spend Down paperwork.

Follows CPST departmental protocols.

Performs problem solving and conflict management Develops and/or implements individualized client treatment or service plans and ensures compliance with applicable standards and agency policies.

Ensures collaboration on all appropriate paperwork, that meets CARF requirements.

Participates in treatment team and treatment POD meetings, and makes recommendations for client status changes.

Completes all client Outcome paperwork in a timely manner.

Operates personal computer to enter, edit and/or retrieve data and to produce reports and correspondence.

Provides staff trainings when appropriateOther duties as assigned
Required Knowledge, Skills and Abilities:
Ability to learn case management principles, healthcare management and reimbursement requirementsExcellent organizational and time management skillsEffective verbal and written communication skillsAbility to relate to and interact with a wide variety of peopleAdvanced level problem solving skills and ability to multi-task while prioritizingAbility to utilize technology including electronic service documentation systemBasic computer skills with ability to navigate Microsoft applications including but not limited to Word, Outlook, Excel, Teams. and
Power Point Job Requirements:
Bachelors Degree in Behavioral Science or related field, or equivalent work experience as allowed/required by regulatory bodiesValid driver license and auto insurance
Essential Functions/Physical Demands:
Positional:
Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.
Gross Mobility:
Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.
Sensory:
Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
