Job Description

Case Manager:
Catholic Charities, Diocese of Cleveland is seeking a case manager to provide direct service to individuals in the WISE Program in Wayne & Holmes Counties. The WISE Program serves individuals with a developmental disability and mental health challenge. The staff member will be trained in Wraparound facilitation. The staff member will provide support, linkages to services and reinforce treatment interventions identified by the client's therapist. This position requires a candidate that loves to be in the community, have a flexible schedule, and enjoys problem-solving with a team!

Program Statement:
The Wayne County Case Manager will work in the WISE Program which provides a team approach to identifying individuals who need extra support to remain in their homes and in the least restrictive environments. The staff member will work with the individual, family and other service providers to identify strengths, needs and provide quality Wraparound care.
What You Will Do:
Work closely with therapists to provide support, community linkages, and reinforce treatment interventions to youth and adults served in Wayne and Holmes Counties. Monitor clients' symptoms and stressors in the home, school or community. Provide crisis intervention as needed. Provide documentation for billing in the Electronic Health Record - AVATAR. Participate in mental health assessments and treatment planning. Update treatment plans and perform discharge summaries in a timely basis as needed. Staff member will facilitate Wraparound meetings and provide in-home support to youth and families in assist with symptom stabilization and increasing interpersonal skills.
