Behavioral Health Case Manager

Mid-Ohio Behavioral Health is seeking a Behavioral Health Case Manager to join our team! Behavioral Health Case Manager's provide a wide range of direct and indirect case management services to consumers with severe and persistent behavior health concerns, including: assessment of strengths, abilities, needs, and preferences; referrals and linkage to resources; advocacy; coordination of services; and assistance with the management of symptoms of behavioral health illnesses. Adherence to client's rights, confidentiality/HIPAA compliance, health and safety issues, and sensitivity to diverse populations is required. Behavioral Health Case Managers' promote a recovery-based model for treatment of consumers that is reflective of the Mid-Ohio's mission statement with the guidance of the clinical director. Applicants are required to have the following: Behaviroal Health Case Manager HS Diploma and Three Years proven experience in the Behavioral Health Care field Basic understanding of MS Word/MS Excel Result driven time management skills Valid drivers license with a clean record Working vehicle with valid auto insurance Must be willing to use personal vehicle for linkage of clients
